MCCH Privacy Policy
How and Why We Obtain Personal Information
MCCH takes great care to protect your Personally Identifiable Information and, when we use it, we do so with respect for your privacy. MCCH Health and Wellness staff may use Personally Identifiable Information about you to develop and deliver services and content that can help you maintain or improve your state of health. We may also use this information to respond to inquiries from you. We may combine your information with that of many other participants to analyze the overall effectiveness of our health promotion programs.
MCCH does not disclose any Personally Identifiable Information to any third party except where such disclosure is required to comply with valid legal requirements such as a law, regulation, search warrant, subpoena or court order; or in special cases, such as in response to a physical threat to you or others, to protect property or defend or assert legal rights. In the event that we are compelled to disclose your Personally Identifiable Information in response to legal action, we will attempt to notify you unless doing so would violate the law or court order.
MCCH collects information about you to assist in your plan for improved health via:
- Online health questionnaires and logs
- Entries made by our staff as they provide service(s) to you
- Information submitted by your physician in response to an Exercise Permission form
- Other interactions with the Motivation web site.
How We Protect Your Information
MCCH has always considered the protection of sensitive information to be a foundation of our service to you. We strive to earn this trust through sound information protection policies so that we may maximize our ability to help you reach your health and wellness goals. We employ extensive physical, electronic and procedural controls and we regularly adapt these controls to respond to changing requirements and advances in technology.
Within MCCH and among our service providers, we restrict access to Personally Identifiable Information to those who require it to provide this service to you. MCCH Wellness and Health Promotion staff are the only ones with rights to access your Personally Identifiable Information. Our service providers are: BSDI, the developer of the Motivation web service, and MaximumASP, the data center management company. Only certified Business Associates at BSDI have access to your Personally Identifiable Information and all are restricted by the terms of this privacy policy. Neither BSDI nor MaximumASP make any use of Personally Identifiable Information.
Privacy Online
Privacy, security and service in our online operations are an essential aspect of the service that we provide. In addition to the personnel restrictions described above, MCCH employs the following Internet-specific practices.
We make extensive use of firewall barriers, encryption techniques and authentication procedures to maintain the security of your online session and to protect your Motivation account from unauthorized access. We may also place cookies and similar files on your hard drive for security purposes, to facilitate site navigation, and to personalize your experience on our site. Our cookies do not identify you by name as an individual and cannot be used to access the Motivation software without your knowledge.
When you visit the Motivation site, we may collect technical and navigational information, such as computer browser type, Internet protocol address, pages visited, and average time spent on our Web sites. This information may be used, for example, to alert you to software compatibility issues, or it may be analyzed to improve our Web design and functionality.
The most important safeguard for your personal information may well be your own security practices. You should never divulge your ID and password to anyone except health and fitness staff at our facility. You should also be sure to sign out of your account when you are finished using it and close the browser window if anyone else might use your computer.
